INSURANCE COVERAGE:
Dr. Hellerstein is a no longer a provider
for any managed care plans. Payment must be made in full at time of visit.
Receipts can be submitted to insurance plans for reimbursement, subject to
the terms of your policy. MISSED AND CANCELLED APPOINTMENTS: Patients will be charged for missed appointments or for appointments cancelled without a minimum of 24 hours notice (24 hour notice includes work days, not weekends). Exceptions to this policy will be made only in cases of life-threatening emergencies |
