INSURANCE COVERAGE: Dr. Hellerstein is a provider for most Oxford Health Care plans, and some (limited) US Health Care plans. In these cases, copayments and deductibles must be paid at the time of visit.  In general, with other insurance carriers, services are provided on a fee-for-service basis. MISSED AND CANCELLED APPOINTMENTS: Patients will be charged for missed appointments or for appointments cancelled without a minimum of 24 hours notice (24 hour notice includes work days, not weekends). Exceptions to this policy will be made only in cases of life-threatening emergencies
